Education & counseling to benefit you.
If you are living with diabetes, you can feel confident in choosing Aspire Rural Health System’s diabetes education programs. Our instructor team includes Certified Diabetes Care and Education Specialists, Registered Dietitian Nutritionists, and a Registered Nurse. Our educators will help you understand how to live with your condition and inspire you to make lifestyle changes that can improve your health.
Once you make the decision to attend our program, you will begin with an individual appointment with one of our instructors. This time is used to discuss your concerns, lifestyle habits, personal health goals, and learning needs. The next step is to attend group classes. Each class is 2-3 hours in length, depending on location, and provides comprehensive coverage of diabetes topics, including:
Our goal is to make you feel confident in your ability to manage your lifestyle and improve your blood glucose levels.
If you have already attended diabetes education, you are likely eligible for annual follow-up education! Two hours of education are available each calendar year for those who wish to refresh their knowledge or seek additional help. These hours can be used to cover whatever topics are most pressing to you, and education can be performed individually or in a group.
For more information about living with diabetes, visit the American Diabetes Association website at www.diabetes.org.
Most people diagnosed with diabetes will qualify to attend diabetes education with a referral from their primary care provider. Most insurance plans cover diabetes education, including Medicare, Medicaid, and many commercial plans. We encourage all patients to verify coverage with their insurance companies for visit codes G0108 and G0109.
Group classes are offered at both our Cass City and Marlette locations. These classes are ongoing and occur at a variety of times throughout the year, including mornings, afternoons, and occasional evenings.
